Privacy notice on the protection of personal data

This privacy notice has been prepared by Luva Özel Sağlık Hizmetleri A.Ş. ("L'UVA"), in its capacity as data controller, under Law No. 6698 on the Protection of Personal Data (KVKK) (the "Law") and the Communiqué on the Procedures and Principles to Be Followed in Fulfilling the Obligation to Inform.

Your personal data we process and the purposes of processing

We process the data below for the following purposes: providing services to you, contacting you when necessary, distinguishing your records from those of other visitors, carrying out transactions, issuing invoices and receiving payments, determining the service you need, providing necessary information, resolving requests and complaints you submit through our communication channels, satisfaction surveys, use as evidence in possible disputes, and purposes arising from our legal obligations under the legislation and towards competent public authorities:

• Identity information (name and surname, Turkish ID number, passport number, date of birth, signature)
• Contact information (address, email address, phone number)
• Transaction information (service contents, fees, payment details)
• Bank account and credit card information

We process your security camera footage to ensure security against conduct contrary to the legislation and to fulfil our obligations to provide information and documents to judicial authorities.

We process your health data (allergies, medical history, past illnesses and surgeries, medications used, physical characteristics such as skin and hair, habits and the like) to assess whether you have an illness, allergy or other health condition that prevents you from receiving a service, and to determine the service suitable for you.

Your data may be stored physically or in electronic environments such as cloud services and data storage systems.

To whom your personal data may be transferred

We may share your personal data with our service providers and infrastructure suppliers in Türkiye and abroad from whom we receive services in information technology or matters requiring expertise; with competent public institutions and organisations and judicial authorities within the scope of our legal obligations and the exercise of our rights; and with business partners we have authorised for the joint provision of certain services.

Method of collection and legal basis

We collect your personal data through automated or non-automated methods such as security cameras, forms you fill in, recording in our systems information you share with us or relating to the service you receive, requests and complaints you submit through our communication channels, and recording of switchboard calls.

We process your data on the legal grounds of "processing being necessary for the establishment and performance of a contract" for the provision of services, "fulfilment of a legal obligation" for retention obligations, and "processing being necessary for the legitimate interests of the data controller" and "processing being necessary for the establishment, exercise or protection of a right" for security camera recordings; we process your health data based on your explicit consent.

Our polyclinic is monitored and recorded by a closed-circuit camera system.

Your rights

You may submit applications regarding your rights under Article 11 of the Law to the data controller Luva Özel Sağlık Hizmetleri A.Ş. in writing to the address below, or by email sent from the email address registered in our system.
